  6. Sign the notarized affidavit assuring that all contractors/subcontractors comply with Kentucky workers' compensation (KRS 342) and unemployment insurance (KRS 341) requirements.
  7. Submit the completed application by mail to P.O. Box 119 or hand-deliver to City Hall.
  8. After permit issuance, coordinate with the Planning & Zoning office to schedule inspections; a Certificate of Occupancy is issued only after the zoning official inspects and signs the form.
