- Permit validity
- Permit becomes void if work does not start within 90 days, if suspended or abandoned for 90 days, if not completed in agreed period, or if changed materially.

- Owner/developer/agent signs; bond may be required at inspector discretion.
- Contractor requirements
- Form captures general, roofing, plumbing, electrical, and HVAC contractor names and Illinois license numbers where applicable.
