- Include truss or rafter uplift load summary sheet, truss layouts, and energy calculations (Form 600A, 600B, or 600C).

- Proof of workers compensation insurance or exemption for contractor and subcontractors
- Permit validity
- Permit becomes invalid if an inspection for permanent construction is not requested within 180 days, or if more than 6 months elapse between inspections.
